What do you make of this quote?

I get the first part, but I don’t understand the last part about geography and all that. Any ideas are greatly appreciated! :]

the last part is talking about how he feels when the morphine gets into him
morphine is a really powerful drug

so he is losing sense of time and reality
by geography it just means he is losing sense of everything
the world and time just seem to not exist the way they did before the morphine
